University of Findlay (UF) cares about the health and well-being of its students, particularly in the current public health environment. All full-time students (9 credit hours for graduate and 12 credit hours for undergraduate), and health professions students without regard to course load*, are required to enroll in the student health insurance plan unless proof of comparable coverage is furnished.

Students must actively attend classes for at least 31 days after the coverage purchase date. Home study, correspondence, and online courses do not fulfill eligibility requirements.

Please note: If you are eligible to purchase coverage in the fall and choose not to enroll before the annual enrollment deadline listed below, you and your dependents will not be eligible to enroll again until the start of the next Fall semester.
UF has contracted with United Healthcare Student Resources (UHCSR) to provide an insurance policy for students who lack valid coverage. The University requires students to annually enroll or waive coverage. In order to waive the University of Findlay Student Insurance Plan, and have the related charge removed from their billing statement, students’ alternate coverage must:

U.S. board certified doctors are available 24/7/365 over the phone or video to resolve a wide range of common and routine illnesses, such as coughs, flu, infections, and more. Maintaining your mental health is very important and your coverage is committed to supporting your holistic well-being. You have access to HealthiestYou mental health providers through a national virtual counseling service. Psychiatrists, psychologists, and licensed therapists are available over the phone or by video chat. There is no cost for using this service.

To access services, please refer to the phone number on the back of your ID card or access My Account at uhcsr.com/myaccount and select My Benefits > Additional benefits > UHC Global Emergency Services.
I don’t have my insurance card! How do I get another one?
Log in to My Account and select the ID card link to see a digital version. You can download, email, or fax your ID card from My Account or UHCSR Mobile App. If you want an actual card, you can request one from My Account tool.
What should I do if I do not feel well or had an accident?
If you feel the sickness or accident is life threatening, call 911 or go to the nearest emergency room immediately. If it is not life threatening, go to Cosiano Health Center. In most cases, it is the most cost effective and easiest to access since they are on campus. If it is before or after hours, you can be seen at Physician’s Plus (3949 N. Main) or Blanchard Valley Hospital (1900 S. Main).
What should I do if Cosiano Health Center is closed or I’m away from school?
If you can’t go to Cosiano Health Center, don’t worry. Our plans include access to a national network of health care providers. You can easily search the network from your school’s Student Insurance page. The network search is also available from My Account and from the UHCSR app. App Store, Play Store.
Preferred providers are those doctors and facilities that have agreed to provide a negotiated discount to members of UnitedHealthcare insurance plans. This may lower your out-of-pocket costs for services received.
I need to check on the status of a claim. How can I do that?
Login to My Account and click on the Claim Summary or My Claims to View Claims. Once in View Claims, you can filter by Patient Name, Provider, or Claim Status. Click on More Options to view Explanation of Benefits or take action on a claim.
Where can I get a prescription filled?
Find the most convenient participating pharmacy by clicking Find Pharmacy on your school’s Student Insurance page. You may also locate participating pharmacies within My Account and the UHCSR app. App Store, Play Store.
I need someone else to call in about my claims. Is that okay?
By law, only the insured person can access their secured healthcare information in My Account. But, if you need someone else to act on your behalf, you can complete and submit a Personal Representative Appointment electronically from My Account or the UHCSR Mobile App.
How much will going to the doctor cost me? Do you have a price estimate?

Why do students need health insurance?
Unexpected health care expenses can jeopardize a student’s financial stability or create barriers toward completing a degree. While all students can receive primary care through the Cosiano Health Center, services needed beyond primary care, such as hospitalization, pregnancy, surgery, or specialty care, are performed off-campus. Health insurance provides students and families with the reassurance that they are prepared for any medical situation.
Will there be a charge for using Cosiano Health Center now?
The Cosiano Health Center is free to all students. The only services charged are for vaccines and the Womankind/Men’s Sexual Health Clinics. Cosiano does not bill insurance.
What is the charge for the student health insurance?
The cost for the annual premium is $2,118.00 which will be billed to your student account ½ in fall and ½ in spring.
How do I know if I have been charged for student health insurance?
You can view your student account in Workday to see the charges. Select the Finances worklet, then select View Account Activity or View Statement.
Can my financial aid help cover the cost of the insurance charge?
Financial Aid can be used to pay for the insurance after tuition, mandatory fees, housing, and meal plans are paid AND you give UF permission to use your aid in this way.
The policy provides coverage for an entire year. What if I graduate in May?
The University of Findlay Student Insurance Plan insures covered students for the full period for which they’ve paid premium, so long as they remain eligible students for at least 31 days after the effective date of coverage. This means that if you withdraw from school for any reason after that period, your health insurance won’t be taken away from you. It also means that if you want to terminate your insurance after that period, you won’t be able to get a refund of premium that you’ve paid. This structure is intended to be fair to students in both situations and is very common in student plans in higher education.
The policy provides coverage for an entire year. What if I am in my home country during school breaks? What if I am studying abroad?
The plan has global emergency services only. Students should purchase other travel insurance if looking for more international coverage while abroad.
What do I do if I forgot to waive but have other valid health insurance coverage?
Students may submit an appeal to the University for review and consideration no later than October 6, 2025. Please forward a copy of your proof of insurance along with a brief explanation of why you feel you should be granted an exception to the UF insurance requirement to the UF Student Health Insurance Plan email (ship@findlay.edu).
Can I add my dependents (such as a child) to the health insurance plan?
Students may not add their dependents, such as their spouse or child, to the student health insurance plan. Other options for dependent coverage may be found at healthcare.gov.
My wavier was denied. Can I appeal this decision?
If you would like to appeal the denial of your waiver, please forward a copy of your proof of insurance along with a brief explanation of why you feel you should be granted an exception to the UF insurance requirement to Skylar Mettert (metterts@findlay.edu). Please note, if your wavier is denied, you will receive an email notification from UHCSR providing the instructions mentioned above.
My need for coverage has changed. Can I enroll mid-year?

Students may inquire about vision and dental plans offered by UnitedHealthcare. To purchase voluntary coverage, please visit uhone.com/uhcsr and select University of Findlay. Please note, vision and dental plans will be at an additional cost for the student and will be charged directly to the student (not through their UF student account).
